1. Your registered email address (xxxxx@xxxxx.com)
2. The current mobile number you wish to close (xxx-xxxxxxx)
3. The new mobile number you wish to use (xxx-xxxxxxx)
4. Your date of birth (DOB) (xx/xx/xxxx)
5. A copy of a phone bill showing your full name and the new phone number on the same page to verify ownership (for prepaid number, please register with your network provider and request this document)
